Vaccination Request Survey / COVID-19
From Today, applications for occupational vaccination have started.
The Niseko Area is considering applying for occupational vaccinations as one entity. We need at least 1,000 people to apply so we would like to ask for your cooperation to complete the following survey so we have a rough idea of how many people would like to receive a vaccination.

①Vaccination Type：Moderna
②Vaccination Date：After June 21st
③Age : over 18 years old
④Staff Family and Business Contacts are also eligible for vaccination
