Occupational Vaccination Application / COVID-19
Thank you for your cooperation with the previous Occupational Vaccination survey. Due to the results of the survey we are expecting to secure at least 1,000 applicants so we are working to proceed with the application. Businesses who wish to have their workers vaccinated with this occupational vaccination, please apply online at the URL below by 3PM June 15th (Tue).
※Please note: The form previously sent was an initial survey, so even if you have filled that in please also complete this registration form as this will be used for the application.
※We also have sent out physical mail but we encourage our members to apply using the form below.
■About the Occupational Vaccination led by our association
Please note that the information and guidance from the Ministry of Health Labor and Welfare may change in the future. ※Please note the conditions for qualifying persons has changed.
Vaccination Starting Date： Early July
Cost ： At this stage we are not expecting our business members to cover any costs.
Vaccination Type： Moderna（Two Vaccination doses 4weeks apart）.
Qualifying Persons：Those who meet all of the following conditions.
・Age over 18 years and under 65 years old.
・NPB・KTA business members staff and their families.
（Business Contact staff and family are also eligible for vaccination）
・Those who work in Kutchan Town or are Kutchan Residents.
Notes： ・Please be careful not to force employees to be vaccinated.
・For further information, we will contact the person in charge at the end of June.
Application Deadline 3PM June 15th （Tue）
