Cancellation Waiting List for the Vaccination
People who have not applied for the vaccination till June 15th can register now for this waiting list.
Occupational Vaccinations by KTA will start on July 2nd (Fri). Applications are now closed. However, we invite people to register for our waiting list in the event that there are cancellations of existing reservations.
■Conditions for this Waiting List
① KTA member / Family
②Residents of Kutchan Town
③Have a phone number that can be called
※We will call you on the day of vaccination.
■Vaccination Flow
①When there is a cancellation, we will count the cancellation numbers in the morning
②In the afternoon, we will start contacting by phone call those who are first on our list（If there is no answer, we will call the next person）.
③If you are available, please go to Hanazono.
Note:
・Please come to the Vaccination Location by 4pm.
・Bring your Identification (Resident Card)
Please wear clothing that allows your upper arm to be exposed quickly to save time.
・(If you can) please bring the 「Prevaccination Screening Questinnaire for COVID-19 Vaccine」 Japanese Form & 「Record of Vaccination for COVID-19」（That is already filled in) https://www.niseko.co.jp/covid19-001

■NOTE
・The below dates are the days that you may get a phone call.July 2nd（Fri）, 6th（Tue）, 9th （Fri）, 13th（Tue）, 16th （Fri）, 27th (Tue).
・The two vaccination dates are fixed and cannot be changed. If you are not available for the 2nd vaccination date, you are not able to take the shot.

④July 13th（Tue）・August 10th（Tue）

・Up to 100 people are able to register for this Cancellation Waiting List. We cannot guarantee you everyone that registers will be able to get the Vaccine.
・People who have applied for the vaccination already cannot register for this waiting list. Please follow this rule as it may cause problems for others.
